Annually, student families are required to complete all necessary forms and pay fees for the upcoming band season. Please review, complete and submit the following forms:
If you are paying by credit card please go to the Jump Start Items Web Page and select items and make relevant payment. If you are paying by check, cash, or money order please print and complete pages 18-19 of the Jump Start Packet and remit with your payment.

Upon the completion/submission of each form, a PDF will be emailed to you containing the information that was entered. Please print, sign, and send back each form.
